<p>Psoriasis is a very common disease. Some people who have not been properly diagnosed by a qualified dermatologist may not realize that they have the disease. There is no research yet to tell us what causes Psoriasis, but so far we know that it is closely related to genetic link. Normally our skin consists of two layers, the top and bottom layers. Every month our skins go through a lifestyle process, a new skin is formed at the bottom layer and slowly pushes to the top layer. The top layer stays alive for a period of 28 to 30 days and gradually gets replaced.</p>
<p>When someone suffers from Psoriasis, this process happens much faster. This cycle happens in only 3 to 4 days. The cells force themselves through the top layer of the skin and the resultant effect is a buildup of dead skin on the surface. This buildup can show up anywhere on the skin surface including the elbow, scalp, knees and knuckles. These patchy areas, which are red in color is known as plaques.</p>

<p>Psoriatic arthritis is a distinct disease, different from rheumatoid arthritis and ankylosing spondylitis- another common type of arthritis- but sharing similar features to each.  The disability and functional impairment in psoriatic arthritis can be as severe as that occurring from rheumatoid arthritis.</p>
<p>Typically, a patient will have had psoriasis for a number of years before the arthritis develops.  A small proportion of patient will develop the arthritis concurrently with the skin disease and an even smaller percentage will develop the skin disease after the joint disease.</p>
<p>Psoriatic arthritis may affect most joints in an oligoarticular pattern, meaning a few scattered joints are affected and the joint inflammation does not have the symmetrical pattern seen in rheumatoid arthritis.  Enthesitis, which is inflammation of the tendons that attach to bone, is common in psoriatic arthritis.  &#8220;Sausage digits&#8221; &#8211; swelling of the fingers and toes so that they look like little sausages is frequent.  Nail changes are also common. These include &#8220;pitting&#8221; and separation of the nail from the nailbed. Some patients will develop carpal tunnel syndrome because of inflammation in the wrist.  Inflammation of the eyes is a serious complication, as is involvement of the aortic valve of the heart.</p>
<p>Joint deformity is frequent and affects 40% of patients with the disease.  Psoriatic arthritis has a significant impact on quality of life.  The skin disease is a tremendous burden and often leads to depression.</p>
<p>The inflammatory process that causes both the skin disease as well as the joint disease is driven by elevated levels of a substance called tumor necrosis factor, or TNF.</p>
<p>A patient with psoriasis who complains of joint pain, swelling, morning stiffness, and fatigue should raise a high level of suspicion for the diagnosis of psoriatic arthritis.</p>

Modern phototherapy devices are many times safer and easier to use than older technology products, delivering exact exposure doses and only a small amount of the UV light spectrum that has been proven beneficial for skin conditions such as psoriasis, vitiligo, acne and dermatitis.</p>
<p>Properly used, UV phototherapy has no proven side effects. Of course, over exposure can cause sun burn, and sun burn is a proven contributing factor related to skin cancers.  There is no clinical evidence to suggest that modern narrow band phototherapy has or does cause skin cancer. A person predisposed to skin cancers by family history, fair skin or other skin conditions should take appropriate precautions while under taking phototherapy, such as covering moles and other skin blemishes and covering their good or clean skin with appropriate sun block or physical blocks such as towels or clothing.</p>
<p>However, the evidence still suggests that phototherapy is significantly safer than uncontrolled over exposure to natural sun light and this can easily be proven here. Sunlight is comprised of many types of light and energy, both visiable and invisable. In the ultra violet spetrum, there are 3 main types of light; UVA (denoted by yellow,) UVB (denoted by green) and UVC (denoted by purple.) The graph below show the break down of natural UV light, narrow band UVB is denoted by Orange.  Think of narrow band UVB as about 1-2% of the UVB spectrum, compared to full sunlight.</p>
<p>UVA sun light</p>
<p>315-400nm	UVB sun light</p>
<p>280-315nm	UVC sun light</p>
<p>100-280nm</p>
<p>UVA light, is found in tanning salons and has the most damaging effect on skin. PUVA treatment for Psoriasis requires additional drugs to force psoriasis to respond to UVA light in the same way as UVB light. Psoralen drugs cause hyper-sensitivity to ALL forms of UV light and visible light meaning your skin will very easily burn in normal sunlight and eyes are extremely sensitive to any form of light.	UVB light, is the medically beneficial component of sunlight, however over exposure still carries some risks.  UVB light (and particularly the narrow band) is the proven component of sunlight that has positive effects on psoriasis, acne, dermatitis and other skin conditions without the need for accompanying drugs.	UVC light, is extremely dangerous but fortunately filtered by the Earth&#8217;s atmosphere and does not reach ground level. If it did, there would be no life on Earth.</p>
<p>PUVA treatment typically involves drugs that cause hypersensitivity to all light, not just UVA light.  The entire UVA spectrum is also typically used for this type of treatment.  Many more times UV radiation than Narrow Band UVB.</p>
<p>When considering any general psoriasis treatment, it is important to also evaluate the following;</p>
<p>o	Known side effects of the treatment</p>
<p>o	Duration treatment has been available</p>
<p>o	Proven track record of the available treatment</p>
<p>o	Duration treatment is required</p>
<p>o	Impact treatment has on daily life</p>
<p>o	Does the treatment best suit your situation</p>
<p>When considering a drug or biological treatment compared with phototherapy, the following facts should be considered;</p>
<p>o	Phototherapy provides long term clearing, effective for 90-95% of patients</p>
<p>o	Widely used for over 40 years</p>
<p>o	Non invasive and can be self administered</p>
<p>o	Costs are often covered by health insurance</p>
<p>o	Little or no ongoing costs, effectively cheaper than years of drug use</p>

<p>A person affected might first mistake it for fungal infection, a disease that also causes white patches to appear on the skin. A big difference between the two is the inflammation of the affected area and the silvery white scale that covers it. These patches are called &#8220;lesions.&#8221; The coverage area of these lesions can range from minimal to widespread depending on the person affected. For most people, though, psoriasis tends to be mild. Unlike fungal infection also, psoriasis is not contagious. Contrary misconceptions, it cannot be passed on to another person by skin contact.</p>
<p>Psoriasis can develop on any part of the body but it commonly appears on the scalp, knees, elbows, and torso. The appearance of lesions is often symmetrical. That&#8217;s why when a lesion appears on the right side of the body, the person affected should expect one to appear on the left also.</p>
<p>Psoriasis rarely appears on children. It usually develops at the age of 15 to 35 and occurs equally both on men and women no matter what race.</p>
<p>Occurrence of psoriasis can range from mild to moderate to severe. When three to ten percent of the body is affected, it is considered a moderate case, but when more than ten percent is affected, it is already considered severe. Whatever the case of psoriasis a person has, though, it always affects his or her quality of living because of the health complications it entails. The skin, which is its primary target, can appear to be discolored if severe psoriasis occurs.</p>
<p>Another health complication that is linked with psoriasis is psoriatic arthritis. It may develop anytime but, mostly, it occurs in the adults. Although having psoriasis does not necessarily mean that psoriatic arthritis will follow.</p>

<p>The term biologics was coined from the word &#8220;bio&#8221; which pertains to living sources such as cells. These types of medicines are unlike traditional drugs which are derived from chemical. Their function is to block or eliminate several immune system cells which contribute to the worsening of psoriasis and psoriatic arthritis. Another systemic treatment is cyclosporine. Originally, it was used on transplant patients in order to avoid organ rejection.</p>
<p>Methotrexate was initially used to treat cancer. During the 1950&#8242;s, though, it was discovered to be effective when it comes to treating psoriasis as well. Soriatane, meanwhile, is an oral retinoid which is currently the only one approved in its league. It is a synthetic form of vitamin A.</p>
<p>While treatments like these have already been discovered and developed, it is also crucial to know what triggers psoriasis. After all, prevention is indeed better than cure. One common and usually ignored trigger is stress. It can either cause psoriasis to appear or to aggravate an existing one.</p>
<p>While relaxation and stress prevention may not be the be-all and end-all cure for psoriasis, it is a great help for affected people both physically and emotionally. Keeping a relaxed disposition can prevent the further worsening of the disease. Also, it can make the affected person remain in control of the situation. One of the best and advanced relaxation methods is hypnosis. While this discipline requires patience, it will help a great deal in improving the disposition of the psoriasis patient.</p>

<p>Psoriasis is a skin disease that occurs when cells in the outer layer of skin reproduce faster than normal and pile up on the skin&#8217;s surface. This produces scaling, itchiness and irritation of the skin.  Psoriasis is non-contagious.</p>
<p>Currently in the United States almost 5 million people are affected by this disease.  It occurs in all age groups and about equally in men and women.</p>
<p>When psoriasis develops, thick patches of skin become red with silvery scales. Often the skin at the joints cracks making outbreak very painful.  Psoriasis most often occurs on the knees, lower back, soles of the feet, scalp, elbows, face and palm.</p>
<p>There have been huge strides in understanding what causes psoriasis.  Recently, a team from the University of Michigan looked for the gene &#8212; called PSORS1 &#8212; in more than 2,700 people from 678 families in which at least one family member had psoriasis.  According to the researchers, PSORS1 is the first genetic determinant of psoriasis to be definitively identified in a large clinical trial. The finding may help in the development of new, more effective treatments for the disfiguring inflammatory skin disease.</p>
<p>Some other research has found that psoriasis may be a disorder of the immune system.   In a normal immune system a type of white blood cell, called a T cell is produced, that normally helps protect the body against infection and disease. Top Scientists have concluded that  an abnormal immune system produces too many T cells in the skin. These excess T cells trigger the inflammation and excessive skin cell reproduction seen in people who suffer with psoriasis.</p>
<p>Doctors usually diagnose psoriasis after a careful examination of the skin. However, diagnosis may be difficult because psoriasis often looks like other skin diseases. A pathologist may assist with diagnosis by examining a small skin sample under a microscope.</p>
<p>For those people who have a suppressed immune system, the symptoms of psoriasis can be extremely severe.</p>
<p>Dietary change can help with psoriasis, avoid alcohol, gluten which is found in wheat, barley and rye, avoid foods that are high in saturated fats, avoid red meats, dairy products, eggs, cheese and sugar and if you are a smoker, then it is best to quit.</p>

<p><b>What Causes Psoriasis?</b></p>
<p>It is very important to understand the psoriasis causes.  While the exact cause of psoriasis is unknown, recent research has shown that psoriasis is caused by a response in the immune system. Scientists believe that T cells trigger an inflammation in the skin, which results in excessive skin cell growth.  T cells are a type of white blood cell that normally helps to protect the body against disease and infection.</p>
<p>It is believed that psoriasis is inherited in many people.  Scientists still don&#8217;t know how or why, but about one third of people with psoriasis have it in their family history.  As an example, the risk of getting psoriasis increases if one parent has it.  The risk is substantially higher if both parents have the disease.</p>
<p>Psoriasis will worsen and improve over time.  For many people, it comes and goes.  Triggers that are psoriasis causes include:</p>
<p>Â·  Infections</p>
<p>Â·  Stress</p>
<p>Â·  Changes in climate that dry the skin</p>
<p>Â·  Certain medications, such as beta-blockers, lithium, or depression drugs.</p>
<p><b>What is psoriasis?</b></p>
<p>Psoriasis is a disease of the skin.  Psoriasis causes scaling and swelling of the skin. Normally, cells grow deep in the skin, and then slowly rise in about a month&#8217;s time, to the surface. <p><b>Psoriasis Treatment</b></p>
<p>Treatment depends upon the severity of the disease, the size of the psoriasis patches, the type of psoriasis, and the reaction of the person to specific treatments.</p>

<p>Therapeutic Use Of Jojoba</p>
<p>Jojoba Oil is a liquid wax that is extracted from the seeds of the jojoba bush. Originally a native of the Sonoran Desert of South West USA and Mexico, its long traditions in Native American uses led to the development of commercial plantations in Israel, the USA, Australia and Argentina.</p>
<p>Dr Mosovich said in his paper: &#8220;About 40 volunteers, in our laboratories, tried using jojoba wax for chapped hands and against sunburn with very good response, and no allergenic reaction. We started our studies on the assumption that jojoba wax could ease certain skin diseases.&#8221;</p>
<p>He also indicated that: &#8220;The known lack of toxicity for human beings induced us to test jojoba oil in two groups of volunteers affected with dermatological processes of high cosmetic and esthetic significance. The first group consisted of 35 patients affected with acne vulgaris. Their condition was &#8216;&#8221;cystic&#8217; or &#8216;purulent&#8217; and seemed to be most resistant to customary treatments.&#8221;</p>
<p>He noted that &#8220;Patients willingly returned as they felt that the facial treatments were useful.&#8221;</p>
<p>&#8220;A second group of 27 patients suffered from psoriasis vulgaris. This condition is characterised by excessive scaling (turn over) and is very resistant to conventional therapeutics. The keratoplastic and keratolytic action of jojoba oil suggested that it might be used as additional treatment. The use of steroids in the treatment of this condition is expensive, cumbersome, and may have secondary effects. Jojoba oil may be recommended in certain cases of psoriasis, such as those affecting the scalp.&#8221;</p>
<p>Treatment of Psoriasis vulgaris</p>
<p>Under this heading, Dr Mosovich&#8217;s paper outlined:&#13;<br/>&#8220;The experience obtained during studies of acne patients, as earlier reported, showed positive keratoplastic and also slight keratolytic effects, ease and convenience of use, and no side effects. These induced us to study the effect of this ointment for treatment of patients suffering from Psoriasis vulgaris. This illness is frustrating for the physician and the patient, is most unaesthetic and is incurable. The treatment which seems to help one patient has no effect on another, and for this reason it is called &#8220;heartbreak disease.&#8221;</p>
<p>&#8220;The psoriasis histological phenomenon consists of parakeratosis, which could affect several areas of the body. The condition follows an irregular cyclic pattern &#8211; improvements followed by declines. The treatment usually used should help to peel the hard scales. Of the medicines used today, some are very expensive and include hormones, such as fluocinonide; the others are classical medicines such as mineral oil or coal tar derivatives, some of which are very greasy, black and, thus, unpleasant for daily use.&#8221;</p>
<p>&#8220;The patients treated: a group of 27 volunteer patients aged 12 to 60 were treated, eighteen of them for psoriasis of the scalp, as other medications used for this illness are not very effective. The illness causes hard itching, open sores and hair clumping.&#8221;</p>
<p>&#8220;Ointment specifications: the lipidic water immiscible ointment contains refined jojoba wax esters in cis and trans forms.&#8221;</p>
<p>&#8220;Method of treatment: The ointment was massaged lightly on the scalp, twice a day, with no immediate hair wash. The patient returns for first control after one week, and for subsequent checks once a month.&#8221;</p>
<p>&#8220;Results: light spreading of the ointment on the scalp or the effected area permits the removal of dandruff or scales, separates the hairs from each other and causes softening of the skin. The area remains erithematosous. However in several of the cases the scales reappear after a time. There are no adverse effects on the skin. In all cases we could positively say that the treatment helped, at least initially, which is not always the case with other classical treatments. (Fig 5 shows marked improvement on the same psoriasis of the elbows case after the first treatment). <p>Psoriasis appears as raised, red patches or lesions covered with dead skin cells on the skin. Scalp psoriasis can be mild, with fine scaling or thick with crusted plaques, covering the entire scalp. The areas of the skin grow very rapidly and form red, scaling patches. Patients experience severe itchiness, a feeling of tightness and soreness, accompanied by hair loss.  Scalp psoriasis is a common form of psoriasis affecting over 50% of patients.</p>
<p>Treatments for scalp psoriasis are generally temporarily effective and continued over a long period. Moderate exposure of the scalp to sunlight and regular washing of the head helps reduce the spread of the condition. Certain shampoo treatments control the spread of psoriasis. Other treatments prescribed by doctors include coal tar, dithranol, salicylic acid, cortico-steroids and Vitamin D derivatives.</p>
<p>The correct method of application of the treatment is important. The hair is parted into sections and the medication is rubbed into the exposed area. It is necessary to comb and brush the hair regularly in order to remove scaling. Shampooing with a coal tar based shampoo followed by a cortisone lotion rubbed into the scalp is recommended.  A frequent break in the regimen ensures that the psoriasis does not become resistant to treatment.</p>
<p>Topical medications like corticosteriods are effective against scalp psoriasis. They are available in solutions, gels, creams, lotions and ointments. A two-week cycle of treatment is commonly recommended for strong steroids.</p>
<p>In cases of mild scalp psoriasis, doctors inject the scalp lesions with steroid medications. The medication is absorbed into the system and is therefore sparingly used.</p>
<p>Systemic medications or prescription medications are administered to patients with moderate to severe scalp psoriasis. Biologic drugs that are made from living human or animal proteins block or eliminate various immune system cells involved in scalp psoriasis.</p>
<p>Since psoriasis is known to be a waxing and waning condition, it usually subsides with regular treatment. It is unusual for anyone to suffer extensive scalp psoriasis for a long time, especially if they seek medical help and use treatments as directed. <p>Psoriasis is a significant problem for dermatologists in the US.  Over 7 million people suffer from this chronic skin condition and it affects both women and men of all ages.</p>
<p>Recent research indicates that Psoriasis may be caused by liver dysfunction and may also be related to immune system functions.  Generally patches of inflamed red skin called plaques characterize symptoms.  While these are the most visible symptoms Psoriasis causes significant discomfort due to pain, itching, limited joint mobility and depending on the affected area, it may also cause emotional distress.</p>
<p>Human blood contains T-cells, which normally protect the body against disease.  Research indicates that in Psoriasis victims these T-cells are over-active and become responsible for excessive skin cell reproduction resulting in the plaques.</p>
<p>Symptomatic treatment of this disease results in destroying the excessive skin production caused by malfunctioning T-Cells.  This is done by use of Ultra Violet light, which can destroy these excessive growths.  The process of destroying these skin cells is called Apoptosis and is included by many doctors as a treatment option.</p>
<p>Many alternative therapies are also available and widely publicized.  These include the use of Dead Sea salts and soaps with high salt content to purging liver treatments.  None of these have been proven conclusively effective in removing the symptoms entirely.  Because of the chronic nature of this condition and its often painful symptoms, patients often tire of non medical intervention and seek help from doctors.</p>
<p>Phototherapy treatments for Psoriasis can be done using UVB and UVA light.  For most mild to moderate cases doctors generally elect to use UVB Narrow Band Spectrum treatments.  Current knowledge of these treatments suggests that these Narrow Band UVB (NBUVB) treatments have better results than Broad Band treatment options.</p>
<p>When UVA light is used, doctors include topical and oral administration of a drug called Psoralen. This drug improves skin sensitivity to UVA light and improves the effectiveness of the treatments.  While the disease may not be entirely curable, such treatments provide significant symptomatic relief to patients suffering from Psoriasis.</p>

After some days, the doctors find out that the person has psoriasis, which is not allowing the skin to heal. The strange behavior of skin at certain places on the body is responsible for psoriasis. <p><strong>Psoriasis &#8211; skin action in psoriasis</strong>  In our skin the bottom layer produces new cells. <p>So as it turns out, this is another one of those annoying, self esteem breakers.  Yes the skin disease Psoriasis.  You read that right it is a disease. A rather embarrassing skin disease that has been known to lead to frustration and in some cases deep depression. If you suffer from it don&#8217;t feel alone, you are just one of 80 million people world wide that suffer with the same skin disease. This makes it a rather common skin disease.</p>
<p>As you can imagine since Psoriasis comes from the Greek word &#8220;psora&#8221; which means &#8220;to itch&#8221;, the skin disease it is well known in living up to its name. As if itching is not bad enough it is sometimes rather painful too.</p>
<p>Views seem to differ in when Psoriasis may occur, yet the most common thought is that Psoriasis can happen at any age between 12 and 45. It is not prone to effect one sex more than another yet it does seem to be more common in adults than in children. Caucasians seem to be at a slightly higher risk too.</p>
<p>Ok, so what is it, well if you suffer from it, or know someone who does you need no introduction, but for those who are not as familiar with it, the most common form of Psoriasis (plaque Psoriasis) is when you find the skin red and inflamed with a silvery white scale on top. These scales gradually enlarge and increase in number. When the scales are scraped off a few bleeding points are visible.</p>
<p>Basically what happens is the skin regenerates too quickly. Skin rapidly accumulates at the effected site which causes the silvery white scales. But my next question was what causes that to happen? Well the blame seems to lie at the doorstep of the T-Cells making it an immune system disorder.</p>
<p>Ordinarily these T-cells are supposed to help protect the body from infection yet in the case of Psoriasis they cause inflammation and rapid production of skin cells. They seem to be trying to heal an injury, one that is not really there. It is still not known what triggers the T-cells to be released to the dermis yet it is known that a normal skin cell matures in 28-30 days but in the case of Psoriasis it can happen as quickly as 3-6 days.</p>
<p>This is not the only form of Psoriasis; there seem to be many variations. People generally only have one form of Psoriasis at a time yet they can change from one form to another. One form of Psoriasis may clear up and later another form will appear.</p>
<p>The other, some refer to it as the more severe form of Psoriasis, is Pustular Psoriasis. These are blisters that are not infectious yet are filled with pus. This pus consists of white blood cells. The skin around this area is red and tender. The blisters eventually turn brown and form a scaly crust.</p>
<p>Guttate Psoriasis derives from the Latin word meaning &#8220;drop&#8221; and they are small red individual spots that usually appear on the trunk and the limbs. There name is because they look like little tear drops. This form of Psoriasis is mostly commonly yet not solely linked with Streptococcal infection.</p>
<p>Inverse Psoriasis is found in the arm pits, under the breasts, and other skin folds around the genitals. This appears as shiny red areas with out the scales that are found with Plaque Psoriasis. It tends to be more common in over weight people as with skin folds and sweating the problem becomes much more irritated.</p>
<p>The general consensus seems to be that even though this disease is not contagious it does seem to be to a large degree hereditary. It is not guaranteed that it will develop from these genes that carry it but there does seem to be certain conditions that may bring it out. Skin damage due to the sun, alcohol, strep throat, stress and some medications (including pain killers and anti malarial medication) may be all that is required to trigger Psoriasis and bring it to the fore. Stress and emotional traumas are rather large contributors to Psoriasis.</p>
<p>It may affect anywhere yet it is most commonly found on the scalp, elbows, knees and lower back. The groin, the nails and the palms and soles of the feet are also affected areas. When the nails become affected they become pitted or discolured. The joints may also be affected and in such cases the Psoriasis is called Psoriatic Arthritis.</p>
<p>In the case of Psoriatic Arthritis the condition is characterized by stiff, tender and inflamed joints. There are five different forms of Psoriatic Arthritis. Generally it only affects a few fingers and toes yet in severe cases this can be rather debilitating to the point that the individual suffering cannot work as all his/her time is preoccupied in trying to alleviate the pain.</p>
<p>Because there is no known cure Psoriasis can be absolutely heart breaking to the person suffering with it. There are medications and therapies that are able to clear up the patches of Psoriasis and to help it go into a state of remission. Treatment varies due to the severity of the Psoriasis and what form of Psoriasis the individual is suffering from. Consulting ones doctor is imperative. It will most likely flare up again over ones life time yet you do not have to give up all hope as the positive thing is that it can be controlled. It is just a matter of finding what works for you.</p>

<p>There are numerous types of psoriasis being the most common plaque psoriasis or Psoriasis Vulgaris that is characterized by red scaly patches of skin that are often covered by silver scales. These patches are extremely painful and they may bleed. Approximately eighty five percent of people suffering with this condition have this type of psoriasis, which can affect the knees, elbows, chest and lower back, as well as the nails and scalp.</p>
<p>Other types of this condition include:</p>
<p>Guttate psoriasis. This affects ten percent of sufferers and the drop-like shapes of scales can cover large patches of the upper torso and may spread to the arms, legs and scalp. &#13;<br/>Pustural psoriasis. This is a rare form of psoriasis that usually attacks the hands and the feet and is characterized by pus-filled pimples. The causes and symptoms of this type of psoriasis differ; it can be caused by the cessation of prescription drugs or as a result of an infection or sunburn. Lithium, a drug used to treat bipolar disorder, as well as certain beta-blockers used to treat high blood pressure, can cause a flare up.</p>
<p>Inverse psoriasis. This is characterized by dry red patches in the folds of the skin and is more common in people who are overweight.</p>
<p>Erythrodermic psoriasis. This type of psoriasis can be extremely serious because it covers a large area of the body, and in some cases, all of it. When the skin becomes inflamed it creates an increase in blood flow which in turn speeds up the heart rate. Since the inflamed skin covers such a wide area the additional strain on the heart is a cause for concern.</p>
<p><b>Treating Psoriasis with Traditional Methods </b></p>
<p>The main goal of traditional psoriasis treatments is to decelerate the growth rate of skin cells and control the condition for long periods of time; it is the fast growth rate of skin cells that is responsible for the red and scaly psoriasis patches.</p>
<p>Traditional treatments include:</p>
<p><u>Topical Treatments</u> &#8211; For a person with mild to moderate psoriasis, a topical approach is the preferred treatment to start with when choosing from the various available psoriasis treatments. &#13;<br/>There are many different strengths in these topicals and the level prescribed will be in accordance with individual needs and responsiveness.</p>
<p><u>Systemic Medications</u> &#8211; These medications are reserved for people who suffer with moderate to severe psoriasis and don&#8217;t respond well to topical medications. These work by suppressing the immune system and reducing inflammation and the rapid growth of cells. Unfortunately, many people experience side effects and discomfort with these medicines.</p>
<p><u>Phototherapy</u> &#8211; Involves exposing the skin to an artificial ultraviolet light source. UVB radiation goes into the skin and slows down the fast growth of skin cells associated with the condition, resulting in clear skin. Studies indicate that the continued use of this treatment may help prolong remission time. However, irritation and sunburn may appear as side effects of UVB treatment and, even though there is no evidence of increased risk of skin cancer from this type of treatment, it&#8217;s important to remember that UVB is a well-known carcinogen in humans.</p>

<p>Treating psoriasis can be frustrating and confusing. No single psoriasis treatment works for everyone, and finding the treatment that works for you can be tricky and time consuming. Fortunately, the options for treating psoriasis are broadening and increasing in effectiveness.  The three most common psoriasis treatments are topical treatments, systemic medications, and phototherapy.  Talking with your doctor, researching your options and sometimes simple trial and error will lead you to the option that is best for treating your psoriasis.</p>
<p>Usually the first step in attempting to treat psoriasis is the use of topical treatments, or products applied to the skin. While some products are available over-the-counter, others are prescribed by your doctor. For some, products such as salicylic acid or tar are effective in controlling the symptoms, but are often ineffective in eradicating the psoriasis. Inflammation, itching, and scaling may be controlled, but many psoriasis sufferers find that the products found in your local drug store are simply not enough. Talk to your doctor about prescription topical ointments, such as Anthralin, Dovonex, or Taclonex if you are not finding relief from over-the-counter psoriasis shampoo&#8217;s or solutions.</p>
<p>For those affected by psoriasis that is non-responsive to topical treatments, systemic medications may be the answer. Usually reserved for those with moderate to severe psoriasis, these prescriptions are used to essentially treat psoriasis from the inside out. Methotrexate, which has grabbed the most media attention recently, is a commonly prescribed medication. Once used to treat cancer, Methotrexate was discovered to effectively treat psoriasis and has been approved by the FDA since the 1970&#8242;s.</p>
<p>Considered one of the more &#8220;extreme&#8221; options in psoriasis treatment, phototherapy is used exclusively in a clinical setting, such as a doctor&#8217;s office or psoriasis clinic. Using wavelengths of ultraviolet light, the skin is exposed to a light source on a set schedule, and with extreme consistency.  Without a consistent schedule, phototherapy cannot effectively treat your psoriasis.</p>
<p>The key to proper psoriasis treatment is constant communication with your doctor. Finding the psoriasis treatment that works for you may not be easy, but never give up. With the many treatment options now available, there is one right for you!</p>

<p>Psoriasis is a chronic, recurring, <strong>non-contagious</strong> skin disorder that is characterized by raised, thickened reddish patches on the skin covered with silvery-white scales that may vary in severity and affect any part of the body, including the nails, scalp and the genital region. There are about 125 million psoriasis sufferers world wide. This makes up 3% of the world&#8217;s total population! It is estimated that six to seven million Americans have psoriasis, and each year 150,000 to 260,000 new cases are diagnosed.</p>
<p>The incidence of psoriasis is much lower in dark-skinned West Africans and African-Americans than in light-skinned persons of European origin. Incidence is also low in the Eskimos and the Japanese, and is extremely rare in Native Americans. Genetic, geographic and environmental factors may play a major role in this striking disparity between the races.</p>
<p>Most psoriasis patients undergo intense emotional turmoil while they fight to control their disease which tends to have periods of flare ups and remissions the year around. Though no cure has yet been discovered, observing certain principles in life style and skin care management will provide patients a scope for optimum control of the disease. Following are a few tips that would be useful for the psoriasis sufferers:</p>
<p><strong/></p>
<p><strong>1. Maintain desirable health habits.</strong> Psoriasis flare ups are common when you are weak or tired. Taking a balanced diet while reducing red meats and alcohol will go a long way in helping the skin to maintain a calm milieu interior. Patients should also drink plenty of water and get at least 7-8 hours of uninterrupted sleep every day. Do moderate exercises at least 3-4 times every week.</p>
<p>Remember that good nutrition, rest and exercise tune up your body and mind and keep up the equilibrium which is essential to reduce the psoriasis flare ups. These healthy habits will also reduce the chances of contracting infection, another important factor in psoriasis aggravations.</p>
<p>I always remind my patients that medications have only about 30% role in the management of psoriasis. Rest of the 70% involves lifestyle adjustments.</p>
<p><strong/></p>
<p><strong>2. Quit Smoking.</strong> <strong/>Many studies have pointed out that pustular psoriasis of the palms and soles, a variant of psoriasis, is aggravated by smoking. Patients who quit smoking found their lesions clearing up faster. Research also suggests that severity of psoriasis may be linked to smoking. Hence it is highly imperative that you totally give up smoking if you want to get rid of those itchy, scaly, red patches.</p>
<p><strong/></p>
<p><strong>3. Avoid alcohol.</strong> Alcohol is a trigger for psoriasis. There is no doubt about it. Though some dermatologists allow moderate consumption of alcohol in psoriasis, it is my personal observation that even a small quantity of alcohol (like a glass of wine or beer) does cause flare ups in patients. Hence the verdict is &#8211; No Alcohol in any forms!</p>
<p><strong/></p>
<p><strong>4. Learn to manage your stress.</strong> Stress has long been linked to the induction and exacerbation of all types of psoriasis. As mentioned above, psoriasis can be considered a life style disease (much like hypertension or increased blood pressure), a combination of genetic predisposition and psycho- neuro-immuno-hormonal triggers playing a significant role in the causation, initial triggering and maintenance of the disease.</p>
<p>Psoriatics should try to learn and master any one of the stress management techniques like progressive relaxation, biofeedback, yoga etc. Counselling sessions and psoriasis support group participations will be beneficial for those who find it difficult to relax by themselves. According to the National Psoriasis Foundation, stress reduction works best when combined with appropriate medical treatment.</p>
<p><strong/></p>
<p><strong>5. Avoid aggravating medications.</strong> Certain medications taken for other diseases can aggravate or even precipitate psoriasis. Medications that can trigger psoriasis are:</p>
<p/>
<ul>
<li>Anti-malarial drugs like chloroquine</li>
<li>Beta-blockers (medication used to treat high blood pressure) and heart medication. </li>
<li>Corticosteroids. Steroids are double edged swords. They can control severe psoriasis quickly, but sudden withdrawal can cause severe flare ups. Personally I avoid prescribing steroids to my patients unless it is absolutely necessary, that is, psoriasis is severely compromising their quality of life. Only after exhausting other treatment modalities do I take up this group of medications and most patients would do well to steer clear of steroids as a first line of treatment for psoriasis. Even topical steroids should be used very carefully and under medical supervision in psoriasis.</li>
<li>Non-steroidal anti-inflammatory drugs like indomethacin worsen psoriasis in most patients.</li>
<li>Lithium , the antipsychotic drug is a known aggravator of psoriasis.</li>
</ul>
<p>If you have a family history of psoriasis or you are suffering from the disease, inform your physician if you are taking any of the above drugs. Your doctor may be able to substitute alternate medication.
<p>6. <strong>Avoid Scratching, rubbing and picking at the lesions.</strong> Any injury to the skin in both involved and uninvolved areas can produce new psoriasis lesions by irritating the basal layer of the skin and switching on the spurting action of the epidermal cells. Research shows that about 50% of people with psoriasis experience the &#8216;Koebner phenomenon&#8217; &#8211; developing a psoriatic lesion at the site of a skin injury. Hence patients should carefully avoid any direct injury to the skin. Scrubbing to remove the scales is a mistake, as this can worsen the disease.</p>
<p><strong/></p>
<p><strong>7. Treat any infectious foci at the earliest. </strong>Studies show that some infections can trigger psoriasis. Streptococcal throat infections often precedes an outbreak of guttate psoriasis(small drop like lesions), especially in children. Inverse or flexural psoriasis is frequently aggravated by a candidal infection in the folds. Severe generalized psoriasis recalcitrant to treatment is seen in immunodeficient conditions like HIV infection. Treating the infection lessens or clears the psoriasis in most cases. Also look for signs of any infectious foci in your mouth(dental caries), throat(tonsillitis), sinusitis, gall bladder(cholecystitis), urinary bladder(cystitis) etc. Finding and treating these infections could give much better control over your psoriasis status.</p>
<p><strong/></p>
<p><strong>8. Treat the disease before it worsens.</strong> Most effective way to stop the itching and appearance of new lesions in psoriasis is to treat the psoriasis properly as soon as the lesions appear. Consult your dermatologist at the earliest sign of a break out.</p>
<p><strong/></p>
<p><strong>9. Take good care of your skin.</strong> Keeping the skin moist and supple is of paramount importance in psoriasis. Any dryness will increase the scaling, flaking and itching which will further aggravate the condition.</p>
<p/>
<ul>
<li>
<strong>Soak in a luke-warm oatmeal bath.</strong> This relieves itching and hydrates the skin layers.</li>
<li>
<strong>Pat your skin dry; do not rub or scrub. </strong>See tip number 6 above. Developing a habit of gently patting your skin dry is advisable.</li>
<li>
<strong>Apply emollients and moisturizers. </strong>Emollients soften the skin. Moisturizers lock in the skin&#8217;s own moisture to prevent dryness and cracking. One of the best ways to lock in moisture is to apply moisturizer after bathing. Regular use of moisturizers can help prevent the itch and pain of dry skin and reduce scaling and inflammation. A urea containing lotion is preferred during the day and a cream or ointment(if you don&#8217;t mind a little messiness) at night to maintain the hydration of the epidermal layers.</li>
<li>
<strong>Use sunscreen.</strong> Patients using retinoids should apply sunscreen 15 to 20 minutes before going outdoors and wear protective clothing. Sun exposure can cause sunburn, which can in turn trigger psoriasis. But complete avoidance of sun can cause more harm than good. A moderate supply of UV rays is indeed good for clearing up of the psoriatic lesions! </li>
<li>
<strong>Wear cotton clothing .</strong>Synthetics will irritate and heat up the skin and worsen psoriasis and are best avoided. If you have to wear them, wear thick cotton undergarments beneath.</li>
<li>
<strong>Look out for winter worsening: </strong>Cold, dry weather worsens psoriasis. Emollient creams and ointments should be used copiously during winter months. Before application of the moisturizer, remember to moist the skin with luke warm water. The use of humidifiers, both at home and work, can add moisture to the air in winter months and help in avoiding exacerbations. Remember that air-conditioning can dry out the skin and aggravate psoriasis just like winter! Keeping the AC to minimum required coolness and regular application of moisturizing creams can overcome this problem.</li>
</ul>
<p><br/><strong/>
<p><strong>10. Learn to live with Psoriasis.</strong> While there is no permanent cure, psoriasis can be successfully managed so that one experiences more good days than bad. Numerous treatment options are available, and recent advances are revolutionizing the management and care of psoriasis. No single treatment will be effective for all patients. Hence a dermatologist will consider a patient&#8217;s overall health, age, lifestyle, and the severity of the psoriasis and then decide on a treatment option that will achieve maximum effectiveness.</p>
<p>Psoriasis patients will do well to keep a positive mental attitude towards life and its happenings. Remember, any negative signal from the mind is an instant trigger for psoriasis!</p>
<p>Psoriasis, though quite a common disease, has not received the attention it deserves from the health authorities in most countries. As a result, the much needed awareness campaigns and funding of research projects have been lagging behind for decades. It was in this context that in 2004 the first World Psoriasis day was launched. Conceived by patients for patients, World Psoriasis Day is a truly global event that sets out to give an international voice to the 125 million psoriasis sufferers.</p>

<p>For many of us psoriasis is a name of some kind of a disease we never really had any interest in, and many people live their whole lives without any knowledge of this disease and its devastating effects and psychological strains. For the 3% of the population that is diagnosed with psoriasis the learning and knowledge of the disease is probably critical from the first day they feel that something is wrong, or they notice some rash or redness on parts of their bodies.</p>
<p>Effecting the scalp, face, arms, fingers, hands, lags, back and the bottom of the feet, psoriasis is one of the most difficult diseases people have to deal with, the lack of awareness to this disease and the people who suffer from it make this even more difficult to bare, and the psychological effects can lead into a road from which there is no return, there are not many health people that can justifiably judge and criticize a psoriasis patient.</p>
<p>Common mostly in adult population this disease almost never appears in childhood or adolescence, the psoriasis disease is considered an immunity system disorder that has significant effect on the skin of the patient. <p>With the massive amounts of information about psoriasis today, it can be intimidating to try and keep abreast of not only new treatments, but their pros and cons.</p>
<p>Without this information, you cannot really make an informed decision about what to discuss with your doctor and what direction you might want to explore in your attempt to control your psoriasis symptoms. One of the newest groups of medicines are biologics, systemic medications (taken orally or injected).</p>
<p>The Latest and Greatest:</p>
<p>Patients with moderate to sever plaque psoriasis have new treatment options, recently approved by the U.S. Food and Drug Administration: alefacept, efalizumab, and etanercept. One of these, etanercept has also received approval for treatment of psoriatic arthritis, giving new hope to these sufferers as well. The new biologics have been projected to be safer than other, older systemic medications, mainly the kidney or liver failure that long-term use of the older medications can cause.</p>
<p>New injected psoriasis treatments include:</p>
<p>1. Alefacept. The original standard treatment of 12 weeks seems to be more effective with longer remission periods when treatment is administered for a longer period of 16 weeks. Side effects (mild and include infections, injection site reactions, itching, and flu like symptoms) show no significant increase with the longer period versus the standard time table. Repeated courses of alefacept seem to be proving safe and effective as well. Aside from the treatment of plaque psoriasis, this medication seems to be effective on palmoplantar psoriasis after 12 weeks of treatment, allowing the patients to walk without pain and use their hands again. It is also safe to begin alefacept treatment while weaning other therapies.</p>
<p>2. Efalizumab. In studies, this psoriasis treatment has shown to be safe and effective for continuous treatment for up to 60 weeks without fear of toxicity. No new side effects were manifested, while the side effects reported in shorter treatment periods did not worsen. These side effects include: headache, chills, fever, and nausea.</p>
<p>3. Etanercept. This biologic treatment for both plaque psoriasis and psoriatic arthritis has been used to treat rheumatoid arthritis and has shown to be safe on long term treatment of these patients. An important factor to note is that there seems to be relapse within three months after discontinuing use in psoriasis treatment although withdrawal does not cause a severe flare of symptoms. Reinstatement of treatment is still as effective without increase in side effects. Unlike treatment methods with Etanercept for rheumatoid arthritis, psoriasis treatment is suggested to be intermittent or rotated with other treatments.</p>
<p>New oral psoriasis treatments include:</p>
<p>1. Fumaric Acid Ester Therapy. Introduced almost 30 years ago, this is one of Germanys most commonly prescribed oral treatments for sever plaque psoriasis. Common side effects include: flushing, diarrhea, abdominal pain, and nausea. Kidney disorders, decreased white blood cell count, and osteoporosis are more serious side effects of this psoriatic treatment. The gastrointestinal side effects have caused 37% of people taking the original medication to discontinue use; however, recent reformulation offers only rare reports of the gastrointestinal side effects.</p>
<p>2. Oral Pimecrolimus. This psoriasis treatment is two pronged, with both incredible results and a terrifying possible side effect. 2003 studies of the drug treatment showed repeated results in completely clearing symptoms of psoriasis within 28 days, fabulous news for psoriasis sufferers. In June 2006, the FDA released a statement relaying the possibility of a dose dependant connection between oral pimecrolimus and lymphoma. This is based on animal studies which were begun as a result of 10 human cases of patients after beginning this treatment. It has yet to be determined whether this is a direct correlation or not and it may take 10 or more years of human studies to determine whether there is a link between this psoriatic treatment and carcinoma. <p>A. Use only as second line agent for short term and intermittent treatment in patients unresponsive to, or intolerant of other treatments.</p>
<p>B. Avoid use in children younger than 2 years of age. The effect on the developing immune system in infants and children is not known. In clinical studies, infants and children younger than 2 years old treated had a higher rate of upper respiratory infections than those treated with placebo cream.</p>
<p>C. Use only for short periods of time, not continuously.</p>
<p>D. Do not use in patients with a weakened or compromised immune system.</p>
<p>E. Use the minimum amount needed to control the patients symptoms.</p>
